We love babies!! My suggestion - bring a friend or colleague on with you for your first couple sessions. We're changing the way users interact with the site to make it more intuitive (and simple), but for now, you'll just choose a condition that you want to work-up (i.e. ab pain 21F - female 21yo) then select "Register as doctor". Your friend will see you in the "DOCTORS LOOKING FOR PATIENTS" section and they'll choose to "PLAY PATIENT" - their script will automatically appear.

If you use this site to practice, please give me your feedback. I used it to prepare for my exam and I think it helped a lot. You should be able to access your consultation videos from your user page, but if you have problems, just let me know and I can arrange them for you. We are basically getting the site ready for its final release but in the meantime, a lot of stuff, we're just doing manually.

Did they work on the site before? Have you tried using either/both on a different application? For example, try Skype, just to make sure that your hardware is working properly. It is possible that (i) log-out, (ii) close Chrome, (iii) reopen Chrome, (iv) login again... may solve the problem. That or a reboot (I assume that was your first thought lol)

I think a common mistake that a lot of people make Is OVERTHINKING!! Pretend you're a monkey. They come in with a major complaint and then you find out some other "problem". It's as simple as saying to EVERY PATIENT, "So, if I understand correctly, you came in because of MAJOR COMPLAINT, and we're also thinking about this OTHER symptom. Is that correct? Is there anything else you wanted to talk about?"

Use that "BLUE SHEET CS MNEMONIC" that everyone seems to have. Memorize it THEN FR CS PUB SAW ID... PAM HITS FOSS

I don't think you'll really need to do rebound tenderness at all. If someone has a real abdomen, you're probably gonna know. You won't need "rebound" to clinch it, because you don't need to clinch. I'm not saying "DON'T DO REBOUND!" but I'm just saying, your goal is to use the short time you have to be very focused and show that you are comfortable with the patient's body, while being respectful.

If it's an appendix, you're gonna know from the history. I don't think you'll need to do much of anything that is uncomfortable for the patient. If they're experiencing knee pain, I'm not really going into a gruelling examination of active and passive ROM in the knee. "Patient exhibited pain and resistance on attempted passive extension" (if that) - "Knee was painful on movement" is probably good enough lol

Basically, YourFirstPatient uses videocam to replicate the experience of taking the Step 2 CS part of the USMLE. A YourFirstPatient tutor will connect with you over video and act as a patient in order for you to practice interacting with patients and diagnosing their illnesses.
